Home Health Fundamentals: The Keys to Success

Whether you have staff that are new to the home health setting or experienced staff that can benefit from a review of their responsibilities under the Medicare and Medicaid home health programs, this one-day program is an important resource for your home health staff.

The program is designed to assist new and experienced home health personnel to understand the interaction between the new expanded Home Health Conditions of Participation, the changing Medicare versus Medicaid eligibility and coverage requirements, appropriate use of patient notices and the vital role of documentation to support the appropriateness and medical necessity for all home health services and ensure that patients receive the best quality of care and the agency remains deficiency free and financially successful.

Home health clinicians are the foundation and front line for the success of your home health program.  They are continually challenged to maintain and demonstrate current knowledge about the many requirements that impact home health services. Unlike other health care settings, the home health clinicians must be knowledgeable about payer eligibility and coverage requirements;  adapt to increasing requirements for interdisciplinary practice in a community-wide setting where there is a myriad of employment arrangements; possess a detailed knowledge of the regulatory requirements for the home health program; and demonstrate excellent communication and clinical practice skills involving a wide variety of age groups, medical conditions, cultures and environmental challenges.  And if those requirements are not enough to deal with, the major reason for denials and citations within the home health setting is inadequate documentation.

Attendees will acquire knowledge of the basic fundamental requirements of Medicare and Medicaid home health programs with regard to:
  • New and expanded requirements under the Conditions of Participation on patient rights, assessment, care planning and coordination that most impact patient care and outcomes along with the documentation necessary to successfully sustain audits and surveys;
  • Discussion of necessary documentation to define and support Medicare eligibility requirements such as homebound status, definition of residence, availability of caregivers, and acceptable orders;
  • Medicaid eligibility including home as the most appropriate place for care;
  • Overview of common coverage questions under Medicare along with the key role of documentation to ensure proper payment and coverage; and
  • Identifying the proper use of the required ABN, HHCCN and NOMNC beneficiary notices.

Consider sending a team of clinicians to this program who can help you lead the clinical culture within your agency to one that is strong in an interdisciplinary, patient centered, outcome-oriented process of assessment, care planning, care provision and coordination to ensure deficiency free audits and surveys.
