During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of Diabetes Best Practice Intervention Package (BPIP) including improvement strategies, tools and resources.   Process for accessing the HHQI University courses of ‘Diabetes Self-Management’ & ‘Diabetes and Your Patients’ (a course for home health aides) will be shared.

Part 2 - Patients with Heart Failure | RECORDING AVAILABLE
During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of the Disease Management:  Heart Failure Best Practice Intervention Package (BPIP) including improvement strategies, tools and resources.  Highlights will include HHQI University courses presented by nationally known speakers titled ‘Exercise Prescription for the Home Health Patient with Heart Failure’ & ‘Maintenance Therapy for Home Health Patients with Heart Failure’ – both of which have been approved for nursing and physical therapy continuing education credits.

Part 3 - Cardiovascular Disease Beyond Heart Failure | RECORDING AVAILABLE
During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of the Preventative Cardiovascular (Aspirin as Appropriate, Blood Pressure Control, Statin Therapy, and Smoking Cessation) BPIPs including improvement strategies, tools and resources.  Additional highlights will include the HHCDR (Home Health Cardiovascular Data Registry) and the multiple HHQI University courses available, including a home health aide course, ‘Heart Attack & Stroke Risk Factors, Signs & Symptoms.

Part 4 - Respiratory Disease | March 20, 2019 

During this webinar, RN Project Coordinators from the Home Health Quality Improvement (HHQI) National Campaign team will provide an overview of strategies and available evidence-based tools and resources for both patients and clinicians to align with your current respiratory disease management program.  Highlights will include discussion of tobacco cessation resources, immunization resources, and unique patient self-management resources.

Presenters: 

Misty Kevech, RN, MS is an RN Project Coordinator for Quality Insights, the Quality Innovation Network (QIN)-Quality Improvement Organization (QIO) for the states of West Virginia, Pennsylvania, Delaware, New Jersey, and Louisiana.  Her responsibilities are with the Centers for Medicare & Medicaid Services’ (CMS) Home Health Quality Improvement (HHQI) National Campaign and involve researching and writing Best Practice Intervention Packages (BPIPs) for home health.  BPIPs provide information, tools, and resources on key topics to improve patient outcomes.  One of Misty’s focus areas is on underserved populations and their specific needs including patients who are in underserved areas, those who are dual-eligible, experience significant health disparities, and those served by small home health agencies.

Cindy Sun, MSN, RN is the Lead Cardiovascular Project Coordinator for Quality Innovation Network (QIN)-Quality Improvement Organization (QIO) for the states of West Virginia, Pennsylvania, Delaware, New Jersey, and Louisiana.  Her responsibilities are with the Centers for Medicare & Medicaid Services’ (CMS) Home Health Quality Improvement (HHQI) National Campaign since 2009.  Her primary responsibilities include organizing the 100+ Network Coordinators, assisting home health agencies with integrating data-driven quality improvement into their QI plans, and assisting with the integration of preventative cardiovascular care into the home health setting through the development of educational resources and the Home Health Cardiovascular Data Registry.
